Position Status: Full Time
Primary Duties: The Medical Social Worker/Social Work Representative is responsible for providing a comprehensive assessment of risk factors for patients and families in order to meet their medically related social and emotional needs. This person will primarily work within our Restorative Care Unit, but will cover other nursing units, as needed. They act as an advocate for both patient and family while developing an appropriate discharge plan. Social work services are available throughout the continuum of care, i.e., Outpatient Services through dismissal from floor care. The social worker also assists with providing information regarding community and financial resources, and may help to provide a link to those services, as appropriate.
Minimum Qualifications: BSW with 2 years experience in a medical or community social work strongly preferred. May consider a bachelors degree in human service field with related experience. MSW is desired, but not required. A minimum of two years social work experience in the field of medical or community social work preferred.
Schedule: Monday through Friday, 8am to 4pm
Week/Holiday Schedule: Rotating weekend on-call; averages 1 weekend per 6 weeks
